CURRENT JOB POSTING: Family Resource and Planning Facilitator                               

We are currently seeking a dynamic, self-motivated individual to join the Brockville and District Association for Community Involvement as a Family Resource and Planning Facilitator. (Full-Time)

The Facilitator will assist families, the community and individuals living with an intellectual disability in ways that will enhance their presence and participation in community life. The Facilitator will assist families, groups, associations, organizations or other community members to develop the required skills and knowledge to enhance the inclusion of people living with an intellectual disability in their communities.

This is a new and unique role focused on strengthening family members served and supporting people to take their rightful place as active contributing citizens. As well as organizing, hosting and facilitating opportunities for families to connect to one another, while identifying and fostering potential family networks, and the broader community conversations and opportunities. The Facilitator will develop a supportive working relationship with families served, remaining flexible and effective in his/her approach.

Some of the duties will include:

  1. Spending time with the whole family and maintaining contact with the families served on a regular basis and supporting them to craft a vision for the future and strengthening them on their journey.
  2. Assisting families to identify their personal supports and services, their recruitment, screening, matching and orientating of their supporters and connections within their community.
  3. Assisting and encouraging families to identify, prioritize and to act upon desired outcomes
  4. Sourcing and supporting opportunities for participation in recreation, volunteerism and employment that are appropriate and valued.
  5. Promoting and supporting families in the development of a personal network that is supportive of their family member.
  6. Sourcing opportunities for learning in natural community settings.
  7. Increasing awareness about the rights of people living with developmental disabilities to be included in community life.
  8. Promoting and supporting new opportunities and/or progressive ideas regarding supports, planning and inclusion.
  1. Fostering and facilitating family groups for the purpose of sharing information, discovering new ideas and strengthening support among families for each other.
  2. Providing families with information about their rights and responsibilities.
  3. Empowering families to advocate on their own behalf.

 

Successful candidates will possess a combination of the following qualifications and experience:

Requirements

  • Post-secondary diploma in the Human Services or related field required or an equivalent combination of education, training and experience.

  • Experience working with individuals living with an intellectual disability in a direct support and/or family support capacity is considered an asset.

  • Ability to communicate effectively both verbally and in writing; while working independently and as part of a team, and perform all duties and responsibilities in a manner consistent with our Values, Mission, Vision.

  • The use of a car with valid Driver’s License and possess a clear police check - Vulnerable Sector.
